Picture order form. *Updated for review.*

I need to develop a form for indivduals to order candid shots of themselves I take after a weekend event.

Do you guys have any that you would mind sharing as a template for me to go off of?

I will do contact sheets out of Aperture and they will select the size, number, etc...

Well this thread did not generate many replies so I decided to go at it; here is what I came up with. Comments on content, layout, presentation, and pricing taken. Thanks guys.
