Is there a way to reset ALL settings to factory defaults? (not just pressing the 2 green buttons, or reset custom shooting bank?) Thanks!

According to the quickstart guide holding down the "Qual" and "+/-" buttons for more than 2 seconds should reset to default settings.

Go to "Shooting Menu" then...see "Reset Shooting Menu"

Note: This restores the camera "default" settings for the CURRENT shooting menu bank...so if you have several menu banks modified, then do this for each menu bank...A, B, C...etc

See page 257 of your manual...

    Hope this helps

so there is no way to restore the camera to an "out of the box" state?

I would reset not only the shooting menu but also the "custom Settings"Take a look at page 398 and 399 in your manual for the list of default settings.
